    NOW HIRING: Registered Nurse RN

    Key Responsibilities of the Role

    As a vital clinical leader on 2nd shift, the Registered Nurse RN provides direct bedside care, directs shift operations, and advocates for resident health and comfort:

  • Comprehensive Clinical Assessment & Care Execution:

  • Perform detailed head-to-toe physical and cognitive assessments of residents on admission, readmission, and upon any acute change in health status.

  • Provide high-quality complex nursing care—including advanced wound care, IV therapy administration, catheter management, tracheostomy care, and pain management routines—in strict accordance with physician orders and state standards.

  • Physician Orders & Pharmacy Coordination:

  • Transcribe physicians' orders accurately into the Electronic Health Record (EHR) system, updating Medication Administration Records (MARs) and Treatment Administration Records (TARs).

  • Communicate directly with attending physicians, medical directors, and pharmacy suppliers to ensure timely order verification, stat medication deliveries, and seamless medication reconciliation.

  • Care Planning & Interdisciplinary Teamwork:

  • Develop, evaluate, and update individualized resident care plans to address physical, nutritional, and emotional health goals.

  • Lead 2nd shift huddles and collaborate closely with physical, occupational, and speech therapists, social workers, dietitians, and attending physicians to ensure holistic care delivery and effective discharge planning.

  • Shift Leadership & CNA Delegation:

  • Direct and supervise Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) on 2nd shift, delegating direct-care duties, monitoring care delivery, and ensuring prompt responses to call lights.

  • Serve as the clinical escalation point during afternoon and evening shifts, managing emergent clinical situations calmly and coordinating emergency transport when required.

  • Documentation & Regulatory Compliance:

  • Maintain meticulous, real-time clinical documentation in the Electronic Health Record (EHR) system, charting status changes, family notifications, physician visits, and treatment outcomes.

  • Rigorously maintain compliance with HIPAA, state department of health regulations, OSHA standards, and facility infection control policies.

    Qualifications

  • Licensure: Must possess an active, unrestricted Registered Nurse RN license in good standing with the State of Wisconsin (or multi-state NLC equivalent).

  • Certifications: Current Basic Life Support (BLS) / CPR Certification required.

  • Clinical Skills: Strong physical assessment skills, proficiency with Electronic Health Records (EHR) platforms (e.g., PointClickCare), precise medication administration techniques, and sound clinical judgment.

  • Teamwork & Leadership: Ability to work independently, manage shift priorities under pressure, and lead a collaborative nursing team with empathy and clear communication.

  • Experience: Prior experience in long-term care, skilled nursing, sub-acute rehabilitation, or hospital environments is preferred, but motivated new graduate nurses with a strong commitment to resident-centered care are warmly welcomed to apply!
