We're looking for a skilled nursing professional with Infection Prevention Certification AND Restorative RN experience who can step into this essential role and make an immediate impact!
WHAT WE'RE LOOKING FOR:
We're seeking a detail-oriented, knowledgeable, and experienced MDS Coordinator (RN) who understands the importance of accurate resident assessments, regulatory compliance, infection prevention, and restorative nursing programs.
REQUIRED QUALIFICATIONS:
Active South Dakota or Compact RN License
5+ years of nursing experience, preferably in Long-Term Care or Skilled Nursing
Infection Prevention Certification – REQUIRED
Restorative RN Experience – REQUIRED
Previous MDS Coordinator experience strongly preferred
Strong knowledge of MDS assessments, Resident Assessment Instrument (RAI) processes, and comprehensive care planning
Understanding of state and federal LTC regulations and documentation standards
Excellent clinical assessment, organizational, and communication skills
Ability to independently manage multiple responsibilities while collaborating with nursing leadership and interdisciplinary teams
Candidates MUST possess Infection Prevention Certification and a Restorative RN background to be considered.

POSITION DETAILS & COMPENSATION
Position: MDS Coordinator – Registered Nurse (RN)
Location: Near Moody County, South Dakota
Setting: Long-Term Care / Skilled Nursing Facility
Contract: 13 Weeks with multiple contracts extensions possible
Schedule: Monday–Friday | 5x8-Hour Day Shifts
Guaranteed Hours: 40 Hours Per Week
Start Date: Approximately 2 Weeks from Offer
Compensation: Earn up to $105,000/year (annualized)
Pay: Weekly Direct Deposit
Benefits: Medical, Dental & Vision
Housing: Facility-provided housing may be available, subject to availability
Extensions: Potential contract extension opportunities
KEY RESPONSIBILITIES
As the MDS Coordinator, you'll play an essential role in ensuring accurate resident assessments, individualized care planning, infection prevention, and restorative nursing services.
MDS Coordination & Resident Assessments
Conduct, coordinate, and complete Minimum Data Set (MDS) resident assessments accurately and within required time frames.
Manage the facility's Resident Assessment Instrument (RAI) process in compliance with state and federal regulations.
Review and verify MDS documentation, supporting assessments, and clinical records for accuracy and completeness.
Collaborate with interdisciplinary teams to develop, implement, and evaluate comprehensive resident care plans.
Ensure documentation accurately reflects residents' clinical needs, functional abilities, and services provided.
Infection Prevention & Control
Develop, implement, and monitor the facility's infection prevention and control program.
Establish and maintain procedures for identifying, preventing, reporting, investigating, and controlling infections and communicable diseases.
Promote infection prevention best practices among nursing staff, residents, and visitors.
Support staff education, infection surveillance, and regulatory compliance initiatives.
Collaborate with facility leadership to maintain a safe, sanitary, and healthy environment.
Restorative Nursing
Plan, organize, implement, and oversee restorative nursing services for residents requiring specialized restorative care.
Coordinate with nursing staff, rehabilitation professionals, and interdisciplinary teams to support residents' functional abilities.
Monitor restorative nursing goals, progress, documentation, and program effectiveness.
Promote resident independence, mobility, safety, and quality of life.
Quality Assurance & Collaboration
Participate in quality improvement initiatives and interdisciplinary care conferences.
Support nursing leadership with regulatory compliance and resident care standards.
Identify opportunities to improve documentation accuracy, resident outcomes, and nursing processes.
