About This Position
Overview
The Sankofa Practice is a relationship-centered primary care clinic founded by Dr. Joycelyn Thomas, guided by the West African principle of Sankofa. We are dedicated to delivering whole person care that respects each patient’s history, culture, and lived experience. Our services include preventive care, chronic disease management, wellness visits, weight management, and family medicine across the lifespan through both in-person and telehealth appointments. We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our primary care team. This role is essential in providing high-quality patient care, fostering strong client communication, and maintaining accurate medical documentation.
Duties
Provide direct patient care in accordance with established clinical protocols and standards.
Communicate effectively with patients to gather health histories, explain treatments, and address concerns with empathy and professionalism.
Utilize Electronic Medical Records (EMR) / Electronic Health Records (EHR) systems to document patient interactions accurately and efficiently.
Assist with medical documentation, ensuring all records are complete, accurate, and compliant with healthcare regulations.
Support the healthcare team in managing chronic conditions and wellness initiatives through patient education and follow-up.
Coordinate with providers to ensure seamless delivery of care during both in-person visits and telehealth appointments.
Maintain a clean, safe, and organized clinical environment in adherence to infection control standards.
Participate in ongoing training and professional development to stay current with best practices in primary care nursing.
Experience
Valid licensure as a Licensed Practical Nurse (LPN).
Prior experience working in a primary care or outpatient clinic setting is preferred.
Proficiency in using EMR/EHR systems for documentation and communication purposes.
Strong communication skills with the ability to connect effectively with diverse patient populations.
Experience in medical documentation, ensuring accuracy and compliance with healthcare standards.
Ability to work collaboratively within a multidisciplinary team while demonstrating compassion and cultural sensitivity.

Pay: $32.69 - $37.50 per hour
Expected hours: 32.0 – 36.0 per week
