Rivers Edge Nursing & Rehab is seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our team in Muscoda, Wisconsin. Responsibilities:

Provide direct patient care under the supervision of licensed nurses, including vital sign monitoring, assistance with daily living activities, and wound care

Collaborate with the healthcare team to develop and implement individualized care plans

Maintain patient confidentiality and adhere to HIPAA regulations

Participate in resident-focused activities and events to promote socialization and engagement

Perform tasks related to patient admission, transfer, and discharge, including data entry and charting

Demonstrate a commitment to our residents' well-being and quality of life
