Licensed Practical Nurse (LPN) – Medical Clinic

Health Concepts is seeking a Licensed Practical Nurse (LPN) to join our medical clinic team. We are looking for a patient-focused nurse who enjoys hands-on clinical work, communicates well, and takes pride in providing quality care in a supportive, professional environment.

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Pay: Based on experience
What You'll Do
Obtain patient histories and record vital signs.
Prepare patients for examinations and assist providers with clinical procedures.
Administer prescribed medications and injections within the LPN scope of practice.
Perform wound care, dressing changes, and specimen collection.
Answer and respond to patient phone calls, communicate clinical messages, and direct questions to the appropriate provider when needed.
Process medication and procedure prior authorizations and coordinate with insurance companies.
Coordinate patient referrals to specialists and assist with scheduling and follow-up.
Monitor patient conditions and report changes or concerns to providers.
Maintain accurate clinical documentation in the electronic health record (EHR).
Provide patient education and follow-up instructions.
Maintain a clean, safe clinical environment and follow infection control and HIPAA privacy requirements.
Qualifications
Current, valid LPN license.
Previous clinical experience preferred.
Strong communication, organizational, and multitasking skills.
Familiarity with medical terminology and electronic health records.
