About This Position

Licensed Practical Nurse (LPN) – Long-Term Care (LTC)

Schedule: Full-time PM shift, 2:00 PM – 10:00 PM Weekend Requirement: Every other weekend Contract Length: 13 weeks Location: New Lisbon, WI

Position Overview

We are seeking a Licensed Practical Nurse (LPN) for a 13-week contract opportunity in a long-term care setting. The LPN will provide resident care, administer medications, monitor changes in condition, communicate with physicians and family members, and assist nursing staff as needed.

Responsibilities

Administer medications according to the five rights of medication administration.

Monitor residents and report changes in condition to the charge nurse.

Notify physicians and family members of changes in resident condition as needed.

Report any suspected abuse, neglect, or misappropriation to the Director of Nursing and Administrator.

Assist CNAs with call lights and other resident care responsibilities.

Perform additional duties as assigned.

Requirements

Active Wisconsin LPN license.

Current BLS/CPR certification.

Long-term care experience preferred.

Strong attention to detail and medication administration accuracy.

Ability to provide holiday coverage as needed.

Additional Information

Full-time afternoon shift, 2:00 PM – 10:00 PM.

Every other weekend required.

13-week contract.

Meditech EMR experience.

Pay: From $40.00 per hour

Expected hours: No less than 40.0 per week
