Shift: Day 5x8-Hour (06:00 - 14:30)
Pay Range: $45/hr - $53/hr

Position Summary:
We are seeking a skilled Licensed Practical Nurse (LPN/LVN) with experience in Long-Term Care (LTC)/ Skilled Nursing Facility (SNF), or Transitional Care Unit (TCU). The ideal candidate will provide compassionate, patient-centered nursing care, administer medications and treatments, monitor patient conditions, manage wounds, and collaborate with the healthcare team to support quality outcomes and patient safety.
Responsibilities Duties:
Provide nursing care to geriatric LTC/TCU patients.
Manage patients with complex long-term and transitional care needs.
Start and maintain IVs and administer IV therapy using IV pumps.
Perform central-line blood draws and dressing changes.
Provide wound care, including wound vac management.
Manage wound drains, JP drains, and Hemovac drains.
Monitor patient conditions and document care in Net Solutions.
Collaborate with PT, pharmacy, social services, radiology, phlebotomy, and other interdisciplinary teams.
Float between LTC and TCU as required.
Experience:
Active LPN/LVN license
BLS Certification
2+ years of LTC/SNF/TCU nursing experience
1+ year of travel nursing experience
IV therapy/Phlebotomy
Central-line blood draws
Central-line dressing changes
IV pumps
Starting and maintaining IVs
Wound care/wound vac experience
Travel experience required; first-time travelers not accepted
Reliable transportation required
Must be willing to float between LTC and TCU
