United Lab Services is seeking a compassionate, qualified, and motivated Nurse Practitioner or Physician Assistant to conduct Annual Comprehensive Health Assessments throughout the Inland Empire.
This is an excellent opportunity for providers seeking flexible, part-time work in a community-based setting without the responsibilities of traditional prescribing and ongoing treatment. You will help patients identify active and chronic health conditions while evaluating their physical, mental, and social health needs.

Position Responsibilities
Conduct Annual Comprehensive Health Assessments to identify active and chronic health conditions.
Evaluate patients’ physical, mental, and social health needs.
Obtain and document vital signs and BMI measurements.
Review current medications and pharmacological therapy.
Perform appropriate physical examinations and health screenings.
Conduct well-child assessments and in-office consultations and services, as assigned.
Educate and counsel patients and families regarding identified health conditions and screening results.
Complete accurate clinical documentation using Practice Fusion EMR.
Follow applicable clinical standards, professional guidelines, and documentation requirements.
Schedule & Work Locations
Job type: Part-time, W-2 employment.
Hourly rate: $65.00.
Expected hours: 20–30 hours per week.
Schedule: 3–4 days per week, Monday through Friday, with occasional Saturdays.
Locations: Corona office, in-home visits, and community centers throughout the Inland Empire.
Assessment duration: Approximately 45–60 minutes per visit, including documentation.
Travel: Mileage and travel time are reimbursed in accordance with company policy.
Patient population: Adults, geriatric patients, and pediatric patients.
Qualifications & Requirements
Active California license as a Nurse Practitioner or Physician Assistant
Current board certification applicable to the provider's discipline.
Professional medical malpractice/liability insurance with minimum limits of $1 million per occurrence and $3 million aggregate.
Valid California driver's license and current automobile insurance.
Knowledge of clinical standards of care and patient assessment practices.
Ability to successfully complete background screening and credentialing.
Strong communication, organizational, and documentation skills.
Spanish-speaking ability a PLUS

Additional Position Details
Employment classification: W-2 employee. Alternative engagement may be considered for providers operating through an LLC with an EIN, subject to company approval and applicable classification requirements.
Contract term: One year, with renewal likely based on business needs and performance.
EMR system: Practice Fusion.
Medical specialties: Geriatrics, Internal Medicine, Pediatrics, and Primary Care.
Relocation assistance: Not provided.
Work setting: In person, including office, in-home visits, and community-based visits.
