We're looking for a confident, independent nurse who enjoys providing skilled nursing care to multiple geriatric patients in their homes daily, managing a full caseload, and completing comprehensive OASIS assessments.
This is NOT a Private Duty Nursing or one-on-one continuous care position!

REQUIRED QUALIFICATIONS
Active Washington or Compact RN License
2+ Years of Skilled Home Health Visiting RN Experience
1+ Year of RECENT OASIS Experience Within the Last 3 Years – REQUIRED
1+ Year of RECENT HomeCare HomeBase (HCHB) Experience Within the Last 3 Years – REQUIRED
Experience completing multiple skilled nursing visits daily with geriatric patients
Proficiency with OASIS assessments: SOC, ROC, Recertifications, Admissions & Discharges
Ability to independently manage a full Home Health caseload
Strong assessment, documentation, and time-management skills
IMPORTANT: Private Duty Nursing, one-on-one continuous care, and facility-only nursing experience do NOT meet the Skilled Home Health Visiting Nurse requirement.
_ POSITION DETAILS & COMPENSATION_
Position: Skilled Home Health Visiting RN
Location: King County - Seattle, WA
Contract: 13 Weeks – Extension Potential
Start Date: ASAP or Within 2 Weeks of Offer
Schedule: Monday–Friday | 8 AM–5 PM
On-Call: Rotation Required If Requested
Productivity: 30 Units Per Week
Patient Population: Primarily Geriatrics
Setting: Skilled Home Health – Multiple Patient Visits Daily
EMR: HomeCare HomeBase (HCHB)
Documentation: OASIS
Compensation: Up to $122,720/Year (Annualized)
Additional Benefits: Mileage Reimbursement, Weekly Pay, Medical/Dental/Vision
WHAT YOU'LL BE DOING
Conduct multiple skilled nursing visits daily in patients' homes.
Complete comprehensive OASIS assessments, including SOC, ROC, recertifications, admissions, and discharges.
Provide skilled nursing assessments, wound care, medication management, and patient education.
Develop and update individualized plans of care.
Maintain accurate, timely clinical documentation using HCHB.
Coordinate care with physicians, therapists, and interdisciplinary teams.
Independently manage daily visits while meeting 30 weekly productivity units.
Participate in an on-call rotation if requested.
