About This Position

Local and Travel RN-Case Manager-Hospice | 13 Weeks Contract

Location: Sunnyvale CA 94085

Shifts Available: Days: 8:30 am-5:00 pm (Weekends: One weekend per month)

Required: 1 Year of Hospice Nursing Experience

POSITION SUMMARY: The Hospice Nurse admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.

Skills

Required: Current registered nurse licensure and nursing experience relevant to case management; patient assessment, patient service, client communication, and electronic health record management skills.

Required: 1 Year of Hospice Nursing Experience

Preferred: Hospice care, home health, managed care, acute care, inpatient, hospital, or post-acute care experience; experience managing patients as a nurse case manager.

Pay: $2,480.00 - $2,720.00 per week
