Requirements & Skills
- Current and active LPN/LVN license in the state of Wisconsin
- Experience or willingness to work in Long Term Care (LTC) settings
- Strong patient care skills including medication administration, wound care, and vital signs monitoring
- Excellent communication and interpersonal skills to effectively interact with residents, families, and healthcare team members
- Ability to follow care plans and document accurately in compliance with regulatory standards
- Reliable and professional with a strong work ethic
- Basic computer skills for electronic documentation preferred
Key Responsibilities
- Provide direct nursing care to residents in a Long Term Care facility according to care plans
- Administer medications safely and accurately, monitoring for side effects and effectiveness
- Perform routine assessments including vital signs and report changes in condition promptly
- Assist with activities of daily living (ADLs) and ensure resident comfort and safety
- Collaborate with interdisciplinary teams including RNs, physicians, and therapists to support resident health goals
- Document all nursing interventions and resident responses in accordance with facility policies and regulations
- Maintain compliance with infection control and safety procedures to promote a healthy environment
- Educate residents and families as appropriate to support understanding of care plans
Qualifications
- Valid LPN/LVN license in Wisconsin
- Previous experience in Long Term Care preferred but not required
- CPR and Basic Life Support (BLS) certification (or willingness to obtain)
- Strong organizational and time management skills
- Ability to work independently as well as part of a multidisciplinary team
Compensation & Benefits
