Join our team as a Registered Nurse (RN - General) in Apple Valley, CA. This is a 13-week travel/contract assignment in SMMC Case Management with day shifts (5x8), guaranteed hours at 36 per week, and an hourly rate of $59.00–$70.00.

Responsibilities
- Provide acute care case management and support discharge planning workflows
- Apply admission criteria to support appropriate placement and care coordination
- Assess benefits eligibility and coordinate next steps for patients
- Coordinate care across the continuum to support safe transitions
- Participate in care coordination activities including disease management support
- Conduct needs assessments and support ordering DME when applicable
- Develop and maintain plan of care documentation
- Support prior authorizations as part of utilization and coordination processes
- Contribute to regulatory-aligned processes including CMS (Centers for Medicare and Medicaid Services), Department of Health requirements, and DRG (Diagnosis Related Groups)
- Align work to The Joint Commission/ Core Measure/National Safety Goals
- Use Epic charting to document case management and related clinical information
- Maintain required checklist documentation
Requirements
- 2 years acute care case management/discharge planning experience
- Patient ratio experience: 1:25
- Epic charting system experience (required)
- Travel experience required (hardstop: no; need a seasoned traveler)
- Acute hospital experience
- Acute hospital specialty experience: Emergency Department
- Acute hospital specialty experience: ICU
- Acute hospital specialty experience: MS
- Acute hospital specialty experience: Surgical Services
