Patient Care Coordinator (LPN/RN) – Office Triage & Chronic Care Management

Community Healthcare Partners (CHP) is looking for a compassionate, clinically skilled, and highly organized LPN or RN to join our healthcare team as a Patient Care Coordinator with a strong focus on office triage and Chronic Care Management (CCM).
This position is ideal for a nurse who enjoys using clinical judgment to assess patient concerns, determine appropriate next steps, coordinate care, and build meaningful relationships with patients managing chronic health conditions. You will serve as a key clinical resource for patients and the healthcare team, helping ensure concerns are addressed promptly and appropriately while supporting ongoing disease management and improved patient outcomes.
What You'll Do
Office Triage & Clinical Support
Serve as a primary clinical point of contact for patients calling or communicating with the office regarding symptoms, health concerns, medications, and treatment plans.
Perform telephone and electronic triage using clinical judgment to assess patient needs and determine appropriate disposition, including self-care guidance, provider appointments, same-day evaluation, or escalation of urgent concerns.
Recognize changes in patient condition and identify situations requiring prompt provider intervention or emergency evaluation.
Gather and document relevant patient information, including symptoms, onset, severity, medications, vital signs when available, and pertinent medical history.
Communicate patient concerns and clinical findings clearly and efficiently to providers and clinical staff.
Respond to patient questions with professionalism, empathy, and appropriate clinical guidance within scope of practice.
Facilitate timely follow-up for abnormal symptoms, laboratory results, diagnostic testing, medication changes, and provider recommendations.
Chronic Care Management
Coordinate Chronic Care Management (CCM) services for an assigned patient panel.
Review patient records and identify patients appropriate for enrollment in CHP's CCM program.
Monitor patients' health status and identify barriers, changes, or gaps in care that may require intervention.
Develop, implement, and monitor individualized care plans in collaboration with patients and providers.
Educate patients regarding chronic disease management, medications, treatment plans, lifestyle modifications, preventive care, and available healthcare resources.
Encourage patient engagement, self-management, medication adherence, and timely follow-up.
Track patient progress and communicate significant changes or concerns to the appropriate provider.
Coordinate care with specialists, pharmacies, home health agencies, durable medical equipment suppliers, and other healthcare partners.
Assist with identifying and addressing gaps in preventive and chronic disease care.
Care Coordination & Office Collaboration
Collaborate closely with physicians, APPs, nurses, medical assistants, scheduling, medical records, billing, coding, and other office personnel to provide coordinated patient care.
Facilitate referrals, appointments, follow-up services, and communication between patients and members of the healthcare team.
Help ensure provider orders, treatment recommendations, and follow-up plans are communicated and completed appropriately.
Maintain accurate, timely, and compliant documentation of all triage encounters, care coordination activities, patient education, and CCM services.
Prioritize and manage multiple patient needs while maintaining a high level of accuracy and attention to detail.
Support quality improvement initiatives and other clinical and office workflows as assigned.
What We're Looking For
Current Nebraska LPN or RN license in good standing.
Strong clinical assessment, critical-thinking, and prioritization skills.
Ability to effectively perform telephone and office-based patient triage.
Excellent communication skills with the ability to obtain pertinent information and communicate concerns clearly to providers.
Strong organizational and multitasking abilities in a busy primary-care environment.
Ability to recognize changes in patient condition and appropriately escalate concerns.
Experience with chronic disease management, primary care, case management, care coordination, or triage is preferred but not required.
Comfortable working independently while collaborating effectively with a multidisciplinary healthcare team.
Strong EMR documentation skills and attention to detail.
Compassionate, patient-centered approach with excellent customer service skills.

LPN: Starting at $28.00 per hour plus CCM Bonus Program
RN: Higher starting wage based on experience, plus CCM Bonus Program
Pay with CCM Bonus Program averages $33–$38/hour
