About This Position

Overview

Location: Sonora, CA

Type: Travel Contract

Shift: Day Shift/Must have EPIC experience/recent hospice experience. We need someone that can work weekends and 3 days during the week. 5 x 8 hrs. days

Position Summary – Licensed Vocational Nurse (LVN) – Home Health/Hospice

Job Summary

Performs specialized nursing procedures and general nursing care for assigned patients under the direction of a Registered Nurse (RN) or Medical Doctor (MD).

Participates in planning and providing patient care.

Uses aseptic techniques for treatments such as sterile dressings, irrigations, and cauterizations.

Provides functional direction to ancillary personnel.

May be required to perform on-call duties based on location and agency operational needs.

Requirements

Education

Preferred: High School Diploma/GED or equivalent.

Preferred: Bachelor's Degree or equivalent combination of education and related experience.

Licenses/Certifications

Required: Licensed Vocational Nurse (LVN) licensure in the state of practice.

Required: Basic Life Support (BLS) Health Care Provider certification from an approved vendor per Adventist Health policy.

Preferred: Arrhythmia certification.

Preferred: Valid Driver's License (DL) and must be at least 21 years of age or older.

Key Responsibilities

Provide care to assigned patients under the supervision of a Registered Nurse (RN).

Perform routine visits according to departmental policies and procedures.

Assist in developing a prioritized interdisciplinary plan of care for each patient.

Implement and coordinate care plans using appropriate organizational skills and reporting mechanisms.

Implement individual teaching plans, including patient teaching protocols and discharge plans, with emphasis on patient decision-making.

Perform clinical procedures including phlebotomy, injections, and drug screens.

Administer medications.

Order and restock supplies according to established par levels.

Maintain neatness and cleanliness of assigned areas.

Perform other assigned job-related duties.
