NFLOC Registered Nurse- Telehealth

We’re currently seeking a RN to join our amazing team.
Details of the Opening:
Anticipated start date: ASAP
Salary Range: $50/hr
Anticipated duration of this contract assignment: 13 weeks (Extensions usually offered)
Anticipated schedule: Monday-Friday

Daily Range of Responsibilities:
Provide person-centered care to frail seniors to support aging in place and independent living.
Help members manage their health and navigate the healthcare system.
Develop and implement individualized plans of care as part of an integrated care team.
Provide telephonic care management and ongoing member support.
Deliver health education and coaching to help members manage chronic conditions.
Monitor members for changes in condition and help prevent symptom exacerbations.
Support interventions designed to reduce avoidable ER visits and hospital admissions.
Coordinate care and resources in accordance with care management, disease management, and complex/enhanced care management programs.
Job Qualifications:
Active, unrestricted California RN license
Bachelor’s degree in Nursing (BSN) required
Previous NFLOC experience required
Telehealth or remote nursing experience preferred
Strong clinical assessment and communication skills
Ability to work independently in a remote environment
Must be available to work PST hours
