About This Position
EveryCare Home Health is seeking a dedicated and compassionate Part-Time Registered Nurse (RN) to join our growing team. This role is ideal for a clinician who thrives in autonomous environments, values one-on-one patient relationships, and wants to play a foundational role in establishing high standards of clinical excellence.
Key Responsibilities
Comprehensive Assessments: Conduct initial and ongoing comprehensive assessments of patient needs, including OASIS data collection (as applicable), physical assessments, and home safety evaluations.
Plan of Care Development: Develop, implement, and regularly evaluate individualized Plan of Care (POC) in collaboration with physicians and the interdisciplinary team.
Patient & Family Education: Educate patients and their caregivers on disease management, medication administration, symptom recognition, and self-care techniques.
Clinical Documentation: Maintain accurate, timely, and compliant electronic medical records (EMR) for every patient encounter.
Quality & Safety Compliance: Ensure all care aligns with state, federal, and agency accreditation standards, actively participating in quality improvement initiatives.
Qualifications & Requirements
Licensure: Active Registered Nurse (RN) license in Nebraska.
Experience: Minimum of 1 to 2 years of professional nursing experience, with at least 1 year of recent home health experience strongly preferred.
Skills: Strong critical thinking, exceptional time-management, and autonomous decision-making abilities. Excellent written and verbal communication skills.
Technology: Proficiency with home health EMR software and electronic documentation.
Requirements: Valid driver’s license, reliable transportation, and current CPR certification.
