Responsibilities:
- Unit case manager
- Conduct comprehensive assessments of patients upon admission
- Collect and review medical records, ensuring accuracy and completeness
- Obtain patient medical history and perform physical examinations
- Collaborate with healthcare team members to develop individualized care plans
- Educate patients and their families on treatment plans and procedures
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and report any changes or concerns to the appropriate healthcare professionals
- Document all patient interactions, assessments, and interventions in medical records
- Adhere to HIPAA regulations to maintain patient confidentiality
Requirements
Current nursing license (LPN or RN) in the state of practice.
Strong knowledge of anatomy and nursing practices related to a skilled nursing facility
Compassionate approach to patient care with excellent communication skills.
Ability to lift patients safely and assist with mobility.
