Overview

Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) - Charge Nurse, where you will play a vital role in delivering exceptional patient care within a fast-paced, supportive environment. As a Charge Nurse, you will lead nursing staff, coordinate patient services, and ensure compliance with healthcare standards. Responsibilities

Provide comprehensive nursing care to patients across various settings, including long-term care, assisted living, and acute care environments.

Lead and supervise nursing staff during shifts, ensuring high-quality patient care and adherence to safety protocols.

Conduct patient assessments, vital signs monitoring, and physical examinations to evaluate health status accurately.

Administer medications, including injections and IV infusions, following established protocols and documentation standards.

Coordinate interdisciplinary communication for discharge planning, case management, and medical documentation updates.

Oversee infection control practices, sterile processing procedures, and ensure compliance with HIPAA and long-term care regulations.

Utilize electronic health record (EHR) systems such as Epic or Cerner for accurate medical records management and documentation.

Assist with diagnostic evaluations, specimen collection/processing, and specialized procedures like catheterization or tube feeding.

Support emergency response efforts by managing airway issues, ventilator use, or acute pain management as needed.

Foster caregiver communication by providing education on dementia care, Alzheimer's disease management, or other chronic conditions.

Experience

Valid Licensed Practical Nurse (LPN) license with current state certification.

Demonstrated experience working with geriatric patients in settings such as skilled nursing facilities or assisted living communities.

Critical care experience preferred, including ICU or emergency room exposure; Level I or Level II trauma center experience is a plus.

Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with ICD-9/10 coding is advantageous.

Strong knowledge of long-term care regulations, dementia/Alzheimer’s/dementia care practices, and pain management strategies.

Experience in medical-surgical units or hospital medicine environments is highly desirable.

Skilled in patient assessment techniques including vital signs measurement and physical examinations; familiarity with respiratory support like ventilators is beneficial.

Excellent communication skills for effective collaboration with healthcare teams and families; experience working with individuals with disabilities or developmental challenges is valued.
