About This Position

Deepening Roots AFH is looking for a caring, dependable, and compassionate Caregiver to join our team at our 3–4 bed Adult Family Home. We provide personalized, 24-hour support to adults with disabilities and other support needs in a comfortable, home-like environment.

This is an excellent opportunity for someone who enjoys building meaningful relationships with the people they support and wants to make a positive difference in their daily lives.

Position Responsibilities

As a Caregiver, you will help ensure our members are safe, supported, respected, and able to live as independently as possible.

Responsibilities may include:

Providing compassionate, person-centered care and daily support

Assisting members with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and personal hygiene as needed

Assisting with mobility, transfers, positioning, and other physical support according to each member's care plan

Supporting members with meals, snacks, hydration, and dietary needs

Assisting with medication-related duties as permitted by training, authorization, and Wisconsin requirements

Encouraging members to participate in activities, community outings, hobbies, and social opportunities

Supporting members in developing and maintaining independence and life skills

Following individual care plans, physician orders, safety plans, and facility policies

Monitoring members for changes in condition, behavior, mood, or health and reporting concerns appropriately

Maintaining accurate daily documentation, including care provided, activities, meals, medications, and significant observations as required

Responding appropriately to emergencies and following emergency procedures

Maintaining a clean, safe, organized, and comfortable home environment

Assisting with laundry, light housekeeping, meal preparation, and other household responsibilities

Protecting member privacy, dignity, rights, and confidentiality

Communicating professionally with members, families, guardians, supervisors, nurses, and other members of the care team

Following all applicable Wisconsin DHS regulations and Deepening Roots policies and procedures

What We're Looking For

We are looking for someone who is:

Compassionate, patient, and respectful

Dependable and responsible

Comfortable working independently and making appropriate decisions

Able to communicate effectively with members and coworkers

Comfortable providing personal care when needed

Able to maintain professional boundaries

Attentive to detail and accurate with documentation

Willing to learn and follow individualized care plans

Comfortable working with adults who may have physical, developmental, behavioral, or medical support needs

A positive team player who genuinely enjoys helping others

Qualifications

Previous caregiving, CNA, CBRF, AFH, group home, or direct-support experience is preferred but not required

Caregiver training and required Wisconsin certifications will be provided as applicable

Must be able to successfully complete required background checks and employment screening

Must be able to complete required training

Must have reliable transportation to and from work

Must be able to perform the physical requirements of the position, with reasonable accommodation when applicable
