JOB DESCRIPTION

Job Title: Admission Coordinator

DEPARTMENT FUNCTION/JOB OVERVIEW

Manages Madonna’s admission process and practices in collaboration with the Director of Admissions. Responsible for daily decision-making regarding appropriateness of patient referrals based on admission criteria, payer source, and resource needs of the organization. Makes daily bed management decisions based on patient appropriateness, regional strategies, payers, internal resources, and core programs. Responsible for maximizing bed occupancy with clinically appropriate patients across both Lincoln campus hospitals. Possesses key critical thinking skills and communication skills in order to collaborate with multiple stakeholders both internally and externally. Work will be performed in an ethical and legal manner following organizational policies, processes, protocols and procedures. Responsible for quality service delivery and internal/external customer relations for Madonna as a whole, including upholding the mission and values for the department and facility.

ESSENTIAL FUNCTIONS

(INFORMATION)

Facilitates daily team admissions meeting and discussions regarding needs of potential patients.

Reviews the results of the clinical pre-admission screening of patients to determine program placement.

Tracks, monitors pre-admission screening information so that clinical information is current facilitating patient admission. Monitors all active and inactive inquiries for both internal and external referrals.

Maintains facility bed availability and program/service information to manage and maximize occupancy levels. Makes determinations regarding which patients are admitted to Madonna's hospital programs.

Assigns patients to Madonna’s inpatient programs in coordination with physicians, case management, nursing management, and program management. Decisions are based upon pre-admission assessment results, program admission criteria, and internal resources.

Arranges for essential equipment, supplies, and services pre-admission to ensure smooth patient transitions.

Communicates daily admission activity to registration and facility staff to assure appropriate room and bed availability facilitating a smooth transition to Madonna.

Manages and resolves any issues regarding daily patient transfers both pre and post admission by coordinating between and finding solutions for clinical liaisons, physicians, nursing, and case management.

Responds to inquiry calls/referrals from hospital discharge planners, physicians, insurance companies, families, and other referral sources per facility guidelines.

Provides all stakeholders with facility related information through facility tours, phone calls, and clinical assessments.

Completes clinical pre admission assessments per paper documentation as needed per request of nurse liaison or Admissions Director.

Maintains a working knowledge of Federal and State regulations and reimbursement (Managed Care, Medicare and Medicaid).

(PEOPLE)

Implements and Instructs and facility staff to regulations, facility programs, pre-admission operational definitions as well as corporate and departmental mission and goals to ensure compliance and delivery of quality service to all internal and external customers.

Collaborates with staff and Director of Admissions on development and ongoing improvement of new and existing Admissions processes.

Conducts routine meetings with facility staff to ensure staff is updated timely and obtains staff input and feedback on prospective admissions.

Encourages the promotion of Madonna by effective communication skills with customers.

Provides family tours as requested.

Follows instruction from supervisor to perform other functions as assigned in order to achieve the goals within the department as well as within the facility.

(THINGS)

Maintain and ensure a safe environment for the department. Handles and operates all necessary equipment and performs required duties according to established safety standards to maintain compliance with regulations and prevent injury.

Must be able to operate admissions/patient software, copier, telephone, fax machine and have basic computer skills, including e-mail, word processing to type, complete patient data, letters and memos.

PHYSICAL DEMANDS AND ENVIRONMENTAL CONDITIONS

Operate a personal computer to access data and information for planning and processing admissions.

Near acuity and accommodation are required for reviewing medical records and charting.

Talking and hearing are essential for taking and giving information over the telephone.

Mobility required for travel to scheduled events or meetings as needed.

QUALIFICATIONS (Education/training and/or Experience)

Must possess a current Nebraska R.N. licensure and a minimum of five years applicable work experience. Experience in rehabilitation setting and CRRN or CM certification preferred. Must possess excellent communication skills, both written and verbal, and have demonstrated ability to work independently and prioritize responsibilities. Valid Nebraska driver’s license and proof of good driving record required.
