Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality, patient-centered care across various clinical settings, ensuring safety, comfort, and optimal health outcomes. Your expertise will support the health and well-being of diverse patient populations, including seniors, individuals with disabilities, and those requiring specialized medical attention. Duties

Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs.

Administer medications and injections accurately following prescribed protocols, including IV infusions and other advanced therapies.

Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.

Manage wound care, catheterization, tube feeding, and sterile techniques to promote healing and prevent infection.

Support discharge planning and coordinate care with physicians, case managers, and family members to ensure seamless transitions across care settings.

Maintain detailed medical documentation within electronic health records (EHR/EHR systems), adhering to HIPAA regulations and confidentiality policies.

Provide patient education on health management strategies, medication administration, and disease prevention tailored to individual needs such as Alzheimer’s care or dementia management.

Skills

Proven experience working with diverse populations including seniors, pediatrics, neonatal or pediatric ventilator management, and individuals with developmental disabilities or behavioral health needs.

Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and clinical assessment techniques.

Proficiency in EMR/EHR systems such as Cerner or Athenahealth for accurate documentation and data management.

Skilled in IV insertion, venipuncture, catheter care, first aid, sterile processing techniques, and infection control practices.

Experience in acute care environments such as ICU or Level I Trauma Centers; familiarity with emergency medicine protocols including airway management and trauma response.

Ability to perform complex procedures like suturing or spinal taps when required; experience in hospice or palliative care is a plus.

Excellent communication skills for effective caregiver communication, patient education, and interdisciplinary collaboration.
