NOTE: This position is specifically for a skilled Home Health RN. Candidates whose experience is limited to Home Care, private duty, sitter care, continuous care, or non-skilled support work may not be the right fit. Qualified candidates must have experience with clinical assessment, case management, OASIS, and home health documentation. An ideal candidate should possess excellent skills in HCHB, OASIS, scheduling, and time management.
WHAT WE ARE LOOKING FOR
We are seeking a compassionate, dependable, and highly organized Home Health RN who thrives in an autonomous environment while providing exceptional patient-centered care in the home setting.
Required Qualifications:
Active RN License Washington or Compact Nursing License
Minimum 2+ years of recent Home Health experience (Geriatrics ONLY)
Homecare Homebase (HCHB) experience
OASIS documentation experience within the last 3 years
Strong clinical assessment and critical thinking skills
Excellent customer service and communication skills
Reliable transportation and strong time management abilities
Professional, compassionate, and patient-focused approach

POSITION DETAILS
Start Date: 2 Weeks From Offer Date or ASAP
Position: Home Health Registered Nurse (RN)
Productivity Expectations: 30 Units Per Week | 6 Points Per Day | 3-5 Patients /Day
Contract Length: 13 Weeks
On Call: Rotation Required
Compensation:
Earn up to $120,620 per year PLUS Mileage Reimbursement
Weekly Pay: Via Direct Deposit Every Friday Morning
Benefits: Medical, Dental, Vision & More (Single & Family Plans Available)
Schedule:
Monday - Friday 8AM-5PM
Productivity Expectations: 30 Units Per Week =40 HRS
Note: Our client is looking for Home Health RN's that can maintain a full caseload of 30 productivity points per week. No more guessing! Full-Time Hours!
RESPONSIBILITIES
Complete comprehensive patient assessments and develop individualized care plans
Manage a home health caseload while maintaining quality outcomes and productivity standards
Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, and OASIS documentation
Coordinate care with physicians, therapists, caregivers, and interdisciplinary teams
Provide patient and family education regarding disease management and treatment plans
Monitor patient progress and communicate changes in condition appropriately
Participate in on-call triage rotation as assigned
Maintain accurate and timely electronic documentation utilizing Homecare Homebase (HCHB)
