About This Position

Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality resident care during evening shifts. The ideal candidate will possess a strong foundation in nursing practices, with experience in diverse healthcare settings including long-term care, assisted living, and acute care environments. As an integral part of our healthcare team, the LPN will deliver comprehensive nursing services, support patient assessments, and ensure compliance with all safety and privacy regulations. This role requires management and oversight of evening cares performed by care staff on the unit. This role offers an opportunity to make a meaningful impact on patient well-being while working with Supportive Leadership and dynamic and Team environment.

Responsibilities

Administer medications, injections accurately according to physician orders and established protocols.

Conduct thorough patient assessments, including vital signs, physical examinations, and documentation of medical histories using electronic health record (EHR) systems such as ECP.

Monitor patients' conditions closely, recognizing signs of deterioration and escalating concerns through triage procedures or emergency response protocols.

Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions, ensuring their safety and comfort.

Assist with complex procedures such as catheterization, wound care, tube feeding, and sterile dressing changes while maintaining aseptic techniques.

Collaborate with interdisciplinary teams—including physicians, case managers, and therapists—to develop and implement personalized care plans.

Support discharge planning and coordinate post-acute or home & community care services for patients transitioning from hospital to home settings.

Ensure compliance with HIPAA regulations by maintaining confidentiality of patient information and proper medical documentation.

Participate in infection control practices and adhere to safety standards to prevent healthcare-associated infections.

Educate patients and their families on health management strategies, medication administration, and disease processes to promote self-care.

Direct and manage resident cares on evenings.

Experience

Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.

Prior experience working in medical-surgical units, long-term care facilities, assisted living communities, or hospital settings such as ICU or emergency departments.

Demonstrated proficiency with ECP for documentation and patient management.

Knowledge of ICD coding (International Classification of Diseases) for accurate medical record keeping.

Experience in specialized areas such as hospice care, geriatrics, trauma centers (Level I/II), infectious disease management, or pediatric care is highly desirable.

Pay: $30.00 - $36.00 per hour
