Responsibilities
- Complete intake and admission screenings (required)
- Perform biopsychosocial assessments
- Provide patient and family education, support, and advocacy (required)
- Work with a multi-disciplinary team to ensure continuity of care
- Collaborate on adoption/foster care support (required)
- Support assessment and planning related to drug/alcohol use and treatment options
- Address suicide/homicide/self-harm concerns
- Support protective services needs, including adult protective services (required) and child protective services (required)
- Assist with emergency detention
- Support guardianship and power of attorney processes as needed (required)
- Assess response to treatment
- Collect and analyze patient information
- Conduct initial evaluations
- Coordinate care (required)
- Facilitate discharge and contingency planning
- Provide crisis intervention
- Coordinate with internal and external team members
- Document pertinent information to facilitate care coordination
- Refer to other disciplines/resources
- Conduct crisis assessments
- Conduct mental health assessments
- Provide crisis intervention/management for teen suicides, drug/alcohol-related issues, and school violence
- Develop clinical assessment reports
- Apply ethical principles
- Identify social, emotional, and behavioral needs
- Identify trauma-related concerns (required)
- Educate patient/caregiver to facilitate discharge (required)
- Provide/reinforce individualized patient/caregiver education (required)
- Conduct clinical work across emergency department and psychiatric settings (required)
- Support care in relation to dementia (required) and psychiatric disorders
- Apply knowledge of cultural issues (required)
- Apply knowledge of community programs, including Medicaid (required) and Medicare (required)
Requirements
- 3 years of experience (required)
- Active certifications: LCSW, LMFT, LMHC (required)
- Ability to chart in Epic (required)
- Protective services experience across adult protective services (required) and child protective services (required)
- Care coordination/case management experience (required)
- Prior experience working with 5150/involuntary holds
- Strong ability to conduct crisis assessments
- Knowledge of adoption/foster care (required)
- Knowledge of suicide/homicide/self-harm risk areas
- Knowledge of emergency detention, guardianship, and power of attorney (required)
