About This Position

Job Summary

We are seeking a dedicated and compassionate Registered Nurse with home health care experience to join our dynamic healthcare team. In this vital role, you will provide personalized nursing care to patients in their homes, ensuring they receive the highest quality treatment and support. Your expertise will help promote recovery, manage chronic conditions, and enhance patient well-being through comprehensive assessments, care planning, and coordination with other healthcare professionals. Duties

  • Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual care needs.
  • Develop and implement personalized care plans in collaboration with physicians, therapists, and other healthcare team members.
  • Administer medications, treatments, and interventions following hospital protocols and health regulation policies.
  • Manage post-acute care transitions by coordinating discharge planning and ensuring seamless continuity of care.
  • Utilize Electronic Health Records (EHR) systems for accurate documentation of patient records, medical histories, and treatment progress.
  • Educate patients and their families on disease management, medication adherence, and health maintenance strategies.
  • Monitor patient progress regularly through diagnostic evaluations and adjust care plans accordingly to optimize outcomes.
  • Ensure compliance with HIPAA regulations to protect patient confidentiality and maintain the integrity of medical records.

    Experience

  • Proven clinical experience in home health nursing or related outpatient settings with a focus on patient service excellence.
  • Critical care experience such as ICU or emergency room background is highly desirable for managing complex cases.
  • Experience working within hospital medicine environments including medical-surgical units or trauma centers (Level I or Level II Trauma Center experience preferred).
  • Knowledge of infectious disease care, hospice services, and behavioral health considerations in a home setting.
  • Familiarity with hospital experience including discharge planning, utilization management, and case management practices.
  • Strong understanding of medical terminology, physiology, and health regulation policies relevant to outpatient and post-acute care.
  • Skilled in managing patients across diverse populations including geriatrics and pediatrics with a focus on client communication and compassionate care delivery.
  • Proficiency in EMR/EHR systems for documentation, intake processes, medical records management, and clinical case management.
