About This Position

Job Summary

We are seeking a dedicated and compassionate LVN to join our dynamic healthcare team as a Case Manager. In this vital role, you will coordinate comprehensive patient care, facilitate effective discharge planning, and ensure seamless communication across multidisciplinary teams. Your expertise in acute and post-acute care settings will drive improved patient outcomes, optimize resource utilization, and enhance patient satisfaction. Duties

Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.

Develop individualized care plans in collaboration with physicians, specialists, and other healthcare professionals to address complex medical conditions such as critical care cases, trauma injuries, infectious diseases, and behavioral health issues.

Coordinate inpatient and outpatient services, ensuring timely access to hospital medicine, emergency medicine, pediatrics, geriatrics, and post-acute care resources.

Manage utilization review processes by evaluating diagnostic evaluations, hospital admissions, and discharge planning to optimize patient care pathways while adhering to HIPAA regulations.

Facilitate communication between patients, families, primary care providers, and multidisciplinary teams to promote understanding of treatment plans and ensure patient-centered care.

Oversee medical records documentation accurately and efficiently while maintaining compliance with healthcare standards.

Assist with intake procedures for new patients requiring hospital or outpatient services; evaluate patient needs based on clinical data and physiological knowledge.

Support hospital staff by providing expertise in medical terminology and nursing practices related to ICU, PICU (Pediatric Intensive Care Unit), OR (Operating Room), Geriatrics, Behavioral health, and emergency room settings.

Participate in diagnostic evaluation processes and coordinate post-discharge follow-up to ensure continuity of care across inpatient and outpatient environments.

Contribute to quality improvement initiatives by monitoring patient service metrics and participating in case management reviews.

Qualifications

Valid LVN license with current state certification required.

Proven experience in critical care environments such as Level II or Level I Trauma Centers is highly preferred.

Strong background in ICU nursing, hospital medicine, emergency medicine, or medical-surgical units.

Experience with post-acute care coordination including discharge planning and outpatient clinic management.

Knowledge of infectious disease care protocols and behavioral health considerations within acute settings.

Familiarity with managed care principles and utilization management processes.

Excellent patient assessment skills combined with a solid understanding of physiology and medical terminology.

Ability to navigate hospital records systems while maintaining HIPAA compliance.

Strong communication skills for effective collaboration with multidisciplinary teams across inpatient and outpatient settings.

Demonstrated ability to handle complex cases involving pediatrics (PICU), geriatrics, or specialized areas such as OR or outpatient clinics is a plus. Job Type: Full-time

Pay: $32.00 - $40.00 per hour
