About This Position
Overview
We are seeking a dedicated and highly skilled Travel Case Management Registered Nurse (RN) to join our dynamic healthcare team in Oklahoma City, Oklahoma. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and hospital medicine, with a strong focus on patient-centered care and case management. This role involves coordinating complex patient care plans across various healthcare settings, ensuring seamless transitions from acute to post-acute care, and optimizing patient outcomes through comprehensive assessment and management. The Travel Case Management RN will collaborate closely with multidisciplinary teams to deliver exceptional service while adhering to healthcare regulations and policies.
Responsibilities
Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine appropriate care plans.
Manage inpatient and outpatient case loads across diverse medical specialties such as ICU, emergency medicine, geriatrics, behavioral health, pediatrics, and post-acute care.
Coordinate discharge planning and transition of care for patients requiring home health services, hospice care, or outpatient follow-up.
Utilize electronic health records (EHR) systems effectively for documentation, medical records management, and communication with healthcare providers.
Collaborate with physicians, social workers, therapists, and other healthcare professionals to develop individualized treatment strategies aligned with patient needs.
Oversee utilization management processes to ensure appropriate resource use while complying with health regulation policies and HIPAA standards.
Facilitate diagnostic evaluations and manage complex clinical cases involving infectious diseases or trauma cases such as Level I or Level II trauma center experiences.
Engage in patient education initiatives to improve understanding of treatment plans and promote adherence to prescribed therapies.
Requirements
Minimum of 2 recent years of acute-care hospital Case Management experience and 2 years of RN experience
Current RN license.
EPIC experience.
Experience with MCG or InterQual (preferred).
CCM certification (preferred, not required).
Strong clinical assessment, care coordination, discharge planning, and communication skills.
Ability to work four 10-hour shifts per week, including a rotating weekend schedule.
Ability to work independently within a multidisciplinary care team.
Willingness to float to other areas, including other Integris facilities, up to 75 miles.
Must possess a valid driver’s license or state-issued ID.
Utility bill in the traveler’s name with matching address within the last three months.
Willing to submit to COVID-19 and flu vaccinations unless exempted for medical/religious reasons.
Interdisciplinary support experience (e.g., physical therapy, social services).
Experience in an acute hospital setting, including ICU, LTAC, Trauma Level I/II, or community hospitals (preferred but not explicitly required).
Certifications such as BLS.
Demonstrated ability in managing complex hospitalized patients and coordinating services.
Comfort working in a high census environment (number of census: 25).
Flexibility to work day shifts and accommodate rotating weekends.
Willingness to accept travel assignment with travel and lodging arrangements.
Must provide 2 supervisory references.
No license hits.
Profile gaps of 3+ months require explanation in work history.
Local candidates within 75 miles are not eligible.
No travel pairs or block scheduling requests accepted.
