About This Position
Overview
We are seeking a dedicated and highly skilled Travel Case Management Registered Nurse (RN) to join our dynamic healthcare team in Jackson, Mississippi. The ideal candidate will possess extensive clinical expertise in acute and post-acute care settings, with a focus on coordinating patient care across diverse environments. As a Travel Case Management RN, you will be responsible for managing complex patient cases, facilitating seamless transitions of care, and collaborating with multidisciplinary teams to optimize patient outcomes. Duties
Conduct comprehensive patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine care needs.
Develop individualized care plans that encompass discharge planning, post-acute care coordination, and home health services.
Manage inpatient and outpatient cases across hospital medicine, emergency medicine, ICU, PICU, and behavioral health settings to ensure continuity of care.
Utilize electronic health records (EHR/EHR management) systems for accurate documentation of medical records and clinical case management activities.
Collaborate effectively with healthcare professionals—including physicians, therapists, social workers, and managed care teams—to facilitate optimal patient outcomes.
Oversee utilization management and diagnostic evaluation processes to ensure appropriate resource allocation and compliance with health regulation policies.
Provide patient education and client communication regarding treatment plans, medication management, and post-discharge instructions.
Ensure compliance with HIPAA regulations and maintain confidentiality of all medical records and sensitive information.
Requirements
Minimum 2 years of Acute Inpatient Medical Case Management experience.
At least 1 year (within the last 3 years) of recent (within the last two years) acute care discharge planning experience for patients with a wide range of medical diagnoses.
Experience in a large busy hospital ED (500+ beds, Level 1/Academic Medical Center).
Prior travel experience in a Level 1 facility within the last 3 years.
Proficiency in EPIC charting.
Strong computer skills.
Willing to submit a license.
BLS Certification required.
Ability to work 8-hour day shifts, Monday through Friday, with no weekend requirements.
Availability to be on camera for a 20-30 minute interview within 72 business hours of submission.
Flexibility to work across inpatient units and coordinate with social work and pharmacy teams.
Willingness to wait for license verification.
Familiarity with rehab, LTC, and Hospice is not required.
Previous travel in a Level 1 facility in the last 3 years.
Possession or ability to acquire proficiency in EPIC.
Ability to adapt quickly to minimal training in unit-specific procedures.
Ability to work efficiently in a high-volume environment with 15-20 patients or consults per day.
