About This Position
Overview
An experienced Registered Nurse (RN) is needed to provide CCM and CoCM support to residents in a nursing home setting. In this role, the RN identifies care gaps such as missing labs or overdue monitoring, coordinates care, and reviews recent provider notes and any hospitalizations to ensure continuity of care. No interventional bedside coverage is required.
Location: Auburn, WA 98002
Setting: Nursing Home
Schedule: Full Time, Monday - Friday. 6.5 -7 hours per day. After hours and weekends not required!
Compensation: $50 - $55 per hour + benefits
Responsibilities
Identify care gaps, assess urgency and coordinate care
Review recent provider notes and hospitalizations to support continuity of care
Pull relevant information from the EMR to strengthen care plan development and documentation
Attend the daily morning meeting to present patient concerns and updates
Engage with and build relationships with the residents, their families and interdisciplinary care teams
Qualifications
Active RN licensure in Washington state
Clinical experience in chronic disease management with knowledge of monitoring and follow-up workflows
Familiarity with CCM and CoCM processes/workflows including patient outreach, tracking outcomes, and engaging patients/families appropriately
Strong care coordination, documentation and care plan experience
