About This Position
MDS Assistant (LPN) | $35–$55/Hour | Edmonds Post Acute | Edmonds, WA
Edmonds Post Acute is hiring an MDS Assistant to support our MDS Coordinator in keeping resident assessments accurate, complete, and on time. You'll use your clinical knowledge to shape care plans, audit charts, and make sure every resident's needs are documented correctly. What You'll Do
Assist the MDS Coordinator with timely, accurate completion of MDS assessments under the RAI process, following state and federal regulations
Help develop and update resident care plans in line with current RAI guidelines and required timeframes
Maintain assessment schedules and track deadlines for MDS submission to the state
Conduct ongoing chart audits to confirm documentation supports assessments and care plans
Audit records for proper physician and family notification, and complete notifications when assigned
Enter assessment data and documentation in our EMR
Attend Interdisciplinary Care Plan meetings and talk with residents and families during care plan reviews
Work with every department to make sure resident needs are captured and communicated

What You'll Need
Active Washington State LPN license (required)
At least 1 year of experience in long-term care or geriatric nursing
Working knowledge of the RAI/MDS process, PDPM, and payer sources (Medicare, Medicaid, HMO, private pay, hospice)
Comfort with EMR systems and data entry; clerical experience is a plus
Strong attention to detail and the ability to manage deadlines
Clear communication skills with residents, families, staff, and outside agencies
Knowledge of HIPAA and a commitment to resident confidentiality
Ability to stand, walk, and move throughout the building, and lift up to 25 pounds
Annual TB screening and physical exam per facility policy
