We are seeking a dynamic and compassionate Resident Care Manager (RN or LPN) to lead and coordinate resident care services within our long-term care or assisted living facility. Responsibilities
Supervise and manage nursing staff and care teams to ensure high-quality patient care aligned with facility policies and healthcare standards
Infection Preventionist
Develop, implement, and monitor individualized care plans for residents, incorporating assessments of medical needs, behavioral health, dementia care, and mobility requirements
Ensure compliance with all state healthcare regulations, HIPAA privacy standards, long-term care regulations, and accreditation standards such as NCQA
Oversee medical documentation, including ICD-10 coding, medication administration records, and nursing notes to maintain accuracy and regulatory adherence
Coordinate with interdisciplinary teams for discharge planning, diagnostic evaluations, medication management, and treatment adjustments
Conduct regular patient assessments including vital signs, physical examinations, and physiological evaluations; respond promptly to acute or emergent health issues
Lead staff training on EMR/EHR systems like Cerner or eClinicalWorks; promote best practices in infection control, sterile processing, catheter care, and aseptic techniques
Requirements
Valid nursing license (RN) with current state licensure; experience in geriatrics or long-term care preferred
Demonstrated leadership experience managing nursing or care teams within a healthcare setting
Extensive knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and documentation standards
Hands-on experience with EMR/EHR systems such as Epic or Athenahealth; proficiency in medical records management
Strong understanding of state healthcare regulations related to long-term care facilities and assisted living environments
Critical care experience including ICU or Level I/II trauma center exposure highly desirable
Skilled in patient assessment techniques including vital signs monitoring, venipuncture, IV insertion/infusion, catheterization, tube feeding management and ventilator support
Ability to communicate effectively with team members, residents’ families, physicians, and external agencies; managerial communication skills essential
Knowledge of infection control protocols and HIPAA compliance; experience working with individuals with disabilities or behavioral health needs is a plus
