Job Overview
We are seeking a dynamic and dedicated Home Care Director / Registered Nurse to join our team with energy and expertise. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring home care services.
Key Responsibilities
1. Leadership & Administration
Mentors and supervises all Home Care Department team members.
Participates in the after-hours on-call rotation and maintains on call calendars.
Addresses home care staff questions and concerns, keeping Executive Director (ED) informed of critical issues. This includes providing leadership and direction.
Develop processes and systems to best deliver quality data/information.
Prepares and delivers annual evaluations to the home care department.
Work with the HR Director for disciplinary and or PIP measures related to the home care department.
2. Clinical Oversight & Training
Leads orientation for home care department regarding personal care services.
Oversees the delegation of medically oriented tasks to unlicensed assistive personnel (UAP).
Ensures direct training to aides, including for Qualified Trainer training.
3. Regulatory Compliance & Quality Assurance
Serves as the primary resource for Wisconsin Medicaid Personal Care regulations and Prior Authorization (PA) processes.
Be knowledgeable and keep current about Federal and State and the Department of Health Services as applicable to Personal Care, Supportive Home Care, including but not limited to: DHS 105.17, 107.112, 133, the Medicaid Personal Care Handbook, EVV and other regulations as required.
Cooperate with the Department of Health and Social Services, funding sources and DQA in investigating compliance with applicable regulations.
Renews applicable contracts, revalidations, and credentialing for the home care department.
Assists in the development and maintenance of the Home Care budget
Communicates regulatory updates to staff and recommends policy changes.
Reviews QI notices related to medical issues and delegated tasks.
Participates in the QA (Quality Assurance) Committee.
4. Case Management & Care Coordination
Secures and renews written physician orders every 3 to 12 months.
Develops personalized care plans considering recipient preferences and conducts supervisory visit reviews at least every 60 days.
Reviews all care plans and supervisory visits from home care department and updates tracking tools.
Reviews and distributes funding authorizations.
Qualifications
Licensed as a Registered Nurse in Wisconsin pursuant to DHS S.50.49 and DHS 105.17.
One year of supervisory experience in personal care services or related program.
Valid driver's license and transportation to perform assigned duties within the five-county service area.
Pay: From $60,000.00 per year
