About This Position
Job description:

Responsibilities:
-Conduct thorough patient assessments and develop individualized care plans.
-Administer medications and treatments as prescribed, including IV infusions and injections.
-Monitor patients’ vital signs and report any changes in condition to the healthcare team.
-Provide education to patients and families regarding health management and treatment plans.
-Collaborate with physicians and other healthcare professionals to coordinate patient care.
-Maintain accurate medical documentation in compliance with HIPAA regulations.
-Perform procedures such as suturing, catheterization, and phlebotomy as needed.
-Manage acute pain effectively through appropriate interventions.
-Participate in discharge planning and case management for patients transitioning from hospital to home or other care settings.
-Ensure adherence to infection control protocols and aseptic techniques.
Rate of pay: $40/hr
Maximum of 3 shifts a week
Must be willing to work PMs on weekdays and any shift on weekends.
AMs: 6:00am - 2:30pm
PMs: 2:00pm - 10:30pm
NOCs: 10:00pm - 6:30am
Holiday: 2 holidays per year at the facility's discretion
Candidates should have a diverse range of experience in nursing, including but not limited to:
-Long-term care experience required
-Familiarity with Point Click Care
-Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation
-Experience working with individuals with developmental disabilities or in hospice & palliative medicine
-Proficiency in performing physical examinations, vital sign assessments, and specimen collection/processing
