About This Position
NOW HIRING: Community-Based Medication Nurse (RN)
Make a Difference Beyond the Walls of a Healthcare Facility!
Are you a nurse who believes that quality care goes beyond administering medications? Are you passionate about mental health, building meaningful relationships, and helping individuals live successfully and independently in their communities?
Dacotah Foundation is looking for a compassionate, organized, and motivated Registered Nurse (RN) to join our Community-Based Medication Delivery Program!
This is a unique nursing opportunity that combines hands-on client care, medication management, community outreach, and team leadership—all while making a meaningful difference in the lives of individuals living with mental health challenges.
What You'll Do
Coordinate Medication Management: Work alongside an LPN to prepare and organize medications for scheduled community deliveries, coordinate prescription changes, and manage medication refill needs.
Provide Community-Based Nursing Care: Deliver medications directly to clients in their homes during scheduled weekday delivery times, building supportive relationships and promoting continuity of care.
Lead and Support a Team: Provide supervision, scheduling, training, and performance oversight for Medication Assistant I (MAI) certified staff responsible for medication delivery.
Promote Medication Adherence: Work directly with clients to identify barriers, encourage consistent medication use, and provide education and support.
Make a Lasting Impact: Help individuals maintain psychiatric stability, reduce the risk of hospitalization, and build the skills and confidence needed for greater independence in the community.
Who We're Looking For
A Registered Nurse who is:
Compassionate and passionate about behavioral health and community-based care.
Organized, dependable, and comfortable managing multiple responsibilities.
Confident working independently while collaborating with a multidisciplinary team.
A natural leader who enjoys mentoring and supporting other staff.
Committed to empowering individuals to achieve stability, recovery, and independence.
Why This Position Is Different
This isn't your typical nursing job!
You'll have the opportunity to develop meaningful relationships with clients, take a proactive approach to psychiatric stability, and lead a team that provides essential services to individuals living independently.

Your nursing skills can make a difference where it matters most—in someone's everyday life.
