Shift & Schedule
- Day Shift (4x10 Hours): 7:00 AM – 5:00 PM
Requirements & Skills
- Minimum of 1 year of experience with outpatient infusion
- Chemo/Hazardous Drug certification (required)
- Valid Oregon Pharmacist License
- Experience in oncology pharmacy (required)
- Proficiency with medication dispensing systems, specifically Pyxis (required)
- Experience with Epic charting system (preferred)
- Community hospital experience (preferred)
- Experience in various work settings including ambulatory care/outpatient clinics and home health infusion (required)
- Intravenous preparation skills, including compounding
- Dosing expertise for high-alert medications, psychotherapeutic management, and therapeutic medication monitoring
- Pharmacotherapy management knowledge, especially related to oncologic medical problems
Key Responsibilities
- Provide expert pharmaceutical care in outpatient infusion and ambulatory care settings, ensuring patient safety and optimal therapeutic outcomes
- Manage chemotherapy and hazardous drug preparation and administration in compliance with regulatory standards
- Utilize Pyxis medication dispensing systems effectively for accurate dispensing and inventory management
- Perform intravenous compounding and preparation of medications following best practices
- Collaborate with multidisciplinary healthcare teams to develop and implement individualized medication therapy plans, particularly for oncology patients
- Monitor and manage dosing of high-alert medications, ensuring safe and effective psychotherapeutic and oncologic treatment regimens
- Document patient care activities utilizing Epic or preferred charting systems, maintaining comprehensive and accurate medical records
- Participate in quality improvement initiatives and maintain compliance with hospital and regulatory policies
Qualifications
- Active Oregon Pharmacist License
- Chemo/Hazardous Drug Certification
- At least 1 year of outpatient infusion experience
- Strong knowledge of oncology pharmacy practices
- Proficiency with Pyxis medication dispensing systems
- Excellent communication, organizational, and teamwork skills
- Ability to work independently and maintain high standards of patient care
