The MDS Nurse supports the MDS Director in completing and submitting Minimum Data Set (MDS) assessments in accordance with state and federal regulations. This role assists with data entry, care plan development, chart audits, and interdisciplinary communication to ensure accuracy, compliance, and timely submissions.

Reporting: This position reports directly to the MDS Director.

Key Responsibilities

Enter and update MDS data accurately in EMR systems

Assist with timely completion of MDS assessments and care plans

Maintain MDS assessment schedules and submission deadlines

Conduct chart audits to ensure proper documentation

Support care plan development according to RAI guidelines

Communicate with interdisciplinary team regarding resident needs

Assist with notification of physicians and families as assigned

Ensure compliance with HIPAA and regulatory requirements

Education and License

High School Diploma or equivalent

Current, active Licensed Practical Nurse, LPN or Registered Nurse, RN license in the state of Washington

Experience (Required)

Minimum 1 year of MDS experience in a skilled nursing or long-term care setting

Qualifications & Skills

Strong knowledge of MDS/RAI process and Case Mix

Understanding of Medicare and Medicaid reimbursement guidelines

Strong documentation and auditing skills

Proficient in EMR systems

Detail-oriented and deadline-driven

Strong communication and teamwork skills

Physical & Working Conditions

Skilled Nursing Facility environment

Frequent standing and walking

Ability to lift up to 25 pounds

Fast-paced, compliance-driven healthcare setting
