Job Summary
Join our dedicated healthcare team as a Home Care Certified Nursing Assistant (CNA), where your compassionate care and clinical experience will make a meaningful difference in patients’ lives. In this vital role, you will provide personalized, in-home support to individuals across a spectrum of needs, from elderly seniors to those recovering from illness or managing chronic conditions. Responsibilities
Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and transfer methods
Monitor and record vital signs, medication administration, and changes in patient condition using paper or electronic charting systems
Support patients with complex medical needs such as nurse delegated tasks
Collaborate with interdisciplinary teams to coordinate care plans, ensuring compliance with HIPAA regulations and maintaining patient confidentiality
Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions, including behavioral health support and safety measures
Assist with medical procedures such as phlebotomy or basic life support (BLS) when trained and certified
Educate patients and family members on health management, medication adherence, and safety protocols within the home environment
Experience
Prior experience working as a CNA in home care settings or long-term care facilities is highly preferred. This position is not open the HCA, NAR, or HHA applicants.
Demonstrated knowledge of geriatrics, senior care, hospice care, or behavioral health environments
Experience working with individuals with developmental disabilities or special needs is advantageous
Certification in CPR and basic life support (BLS) is mandatory along with Food Handlers
Strong communication skills to effectively engage with patients, families, and healthcare teams while demonstrating compassion and professionalism
You must be authorized to work, be able to pass a background check, and have at least two verifiable references (non-family)
