What You'll Do
Provide skilled nursing care in patients' homes under the direction of an RN.
Monitor patient conditions and communicate changes to the care team.
Follow individualized Plans of Care.
Provide treatments and nursing services as ordered.
Educate and support patients and families.
Complete timely clinical documentation.
Collaborate with the interdisciplinary care team.
What You'll Need
Current, active LVN/LPN license
Graduate of an accredited LVN/LPN program
1+ year of clinical nursing experience
Home health experience preferred
Current CPR certification
Valid driver's license and reliable transportation
Auto liability insurance
