About This Position

Job Overview We are seeking a compassionate and dedicated LPN to join our team. The ideal candidate will have experience in long-term care settings, such as nursing homes or assisted living facilities, and will be committed to upholding resident rights while providing high-quality care. As an Director of Resident Services, you will play a crucial role in managing patient care and supporting the operations of our healthcare facility.

Responsibilities

Provide direct nursing care to residents in accordance with their individual care plans.

Monitor and document residents' health status, including vital signs and changes in condition.

Oversee medications and treatments as prescribed by physicians.

Oversee the care staff that assist with daily living activities, ensuring the comfort and dignity of each resident.

Conducting assessments for acute concerns

Collaborate with healthcare team members, including social workers and registered nurses, to develop and implement care plans.

Educate residents and their families about health management and wellness practices.

Uphold resident rights and ensure a safe, respectful environment for all individuals in the facility.

Participate in operations management by maintaining accurate records and adhering to facility protocols.

Adhere to DHS guidelines ensuring facility compliance in all aspects.

Helping with Nursing on call for after hours

Hours are Monday - Friday, 8a-12pm

Qualifications

Valid LPN license in the state of practice.

Experience in long-term care, nursing homes, or assisted living preferred.

Knowledge of resident rights and commitment to advocating for patients' needs.

Strong communication skills to effectively interact with residents, families, and healthcare team members.

Ability to manage multiple tasks while maintaining attention to detail.

Familiarity with social work principles is a plus but not required.

Compassionate demeanor with a genuine desire to provide quality care.
