Job Title: Licensed Practical Nurse (LPN) – Long-Term Care
Location: New Lisbon, WI
Job Type: 13-Week Contract
Facility Type: Long-Term Care (LTC)
Shift: Full Time PM Shift (2pm-10pm) Every Other Weekend
Job Responsibilities:
Provide direct nursing care to residents in a long-term care setting.
Administer medications and treatments as ordered by physicians.
Monitor residents’ health conditions and report changes promptly.
Assist with activities of daily living (ADLs) and provide compassionate resident care.
Complete accurate nursing documentation and maintain patient records.
Collaborate with RNs, CNAs, physicians, and other healthcare professionals.
Follow facility policies, infection-control procedures, and safety standards.
Respond appropriately to resident needs and emergencies.
Requirements:
Active Wisconsin LPN license.
Previous LTC/SNF experience .
BLS certification.
