Envision Hospice is looking for a compassionate Licensed Practical Nurse (LPN) to join our Kitsap County team. In this role, you'll provide hands-on nursing care and support to hospice patients in their homes, skilled nursing facilities, and other care settings. You'll work closely with our interdisciplinary team to help patients remain comfortable while providing education and support to their families.

What You'll Do
Provide skilled nursing care to hospice patients in accordance with physician orders, hospice policies, and regulatory standards
Administer medications, monitor patient conditions, and document care accurately
Support symptom management and patient comfort
Educate patients and families about medications, comfort measures, and the plan of care
Collaborate with RNs, hospice aides, social workers, chaplains, and other members of the hospice care team
Respond promptly to patient and family needs, including symptom management and urgent care situations
Maintain accurate documentation and follow hospice policies, state regulations, and Medicare guidelines
Participate in the weekend on-call rotation as scheduled
Requirements:
Current Washington State Licensed Practical Nurse (LPN) license
Current BLS certification
Hospice, home health, or palliative care experience preferred
Strong clinical assessment and critical-thinking skills
Excellent communication and interpersonal skills
Ability to work independently while remaining connected to the interdisciplinary team
Valid driver's license and reliable transportation
