About This Position
Countryside Manor is looking for an experienced long term care Licensed Practical Wellness Director to oversee the clinical care for seniors who reside at our community.
The LPN Wellness Director is a key component to providing care and services to the residents in a home-like environment in which they receive individualized services based on their needs and preferences. The Licensed Practical Nurse assists the RN Wellness Director with the medical components related to their care and services.
Flexible scheduling
Part-time to include an every other weekend rotation
Job duties include but not limited to:
Assessments
Care Planning (Development and Meetings)
Coordinating Care and collaboration amongst internal teams, physicians, and third party providers
Policy development and implementation
Employee education and training
Oversight of medication management program
Quality assurance
Completing medication reviews
Resident education
Must be a licensed nurse in the State of WI
On call for staff to report and triage (rotates on weekends)
Job Type: Part-time
Pay: $36.00 - $38.00 per hour
