Join Our Team as a Registered Nurse – Case Manager in Santa Monica, CA

We are actively seeking a dedicated and experienced Registered Nurse to join our team as a Case Manager. Shift & Schedule

  • Day Shift: Monday through Friday
  • Hours: 8:00 AM – 4:30 PM (5 x 8-hour shifts)

Requirements & Skills

  • Minimum 2 years of acute care hospital inpatient Case Manager experience performing Care Coordination and Admission Reviews
  • Experience must be current within the last 6 months
  • Proficiency with Epic electronic charting system is required
  • Seasoned traveler with travel experience required; candidates will be screened accordingly
  • Work experience in acute hospital settings
  • Comprehensive knowledge and experience with:
    • Admission Criteria assessment
    • Benefits Eligibility determination
    • Care Coordination and Discharge Planning processes
    • Needs Assessment and ordering durable medical equipment (DME)
    • Plan of Care development and management
    • Prior Authorization processes
    • Utilization of InterQual Criteria for clinical decision-making

Key Responsibilities

  • Perform in-depth admission reviews and coordinate care for inpatient acute care patients
  • Collaborate effectively with multidisciplinary teams to create and implement comprehensive plans of care
  • Conduct thorough needs assessments and facilitate the arrangement of necessary resources, including DME
  • Manage discharge planning to ensure seamless patient transitions to appropriate levels of care
  • Ensure compliance with admission criteria and benefits eligibility requirements
  • Complete accurate and timely documentation in Epic system
  • Initiate and follow through with prior authorizations to secure necessary services for patients
  • Apply InterQual Criteria consistently to support clinical decision-making and utilization review
  • Adapt to varying environments with travel as required, demonstrating flexibility and professionalism

Qualifications

  • Current Registered Nurse (RN) license in good standing
  • Minimum 2 years of acute care inpatient case management experience
  • Proficient with Epic electronic health record system
  • Strong understanding of case management principles, utilization review, and care coordination
  • Excellent communication, organizational, and critical thinking skills
  • Ability to manage multiple priorities in a fast-paced healthcare setting
  • Willingness and ability to travel as required for assignments
