Responsibilities
- Support psychiatric hospital services, including 5150/involuntary holds.
- Complete intake and admission screenings and perform biopsychosocial assessments.
- Provide patient and family education, support, and advocacy.
- Work with the multidisciplinary team to ensure continuity of care.
- Apply knowledge of adoption and foster care, drug and alcohol use and treatment options, suicide, homicide, and self-harm.
- Apply knowledge of adult protective services, child protective services, emergency detention, guardianship, and powers of attorney.
- Assess response to treatment, collect and analyze patient information, and conduct initial evaluations.
- Provide care coordination and case management.
- Conduct crisis assessments and mental health assessments.
- Provide crisis intervention for teen suicides, drug- and alcohol-related issues, and school violence, including crisis intervention and management.
- Develop clinical assessment reports while following ethical principles.
- Identify social, emotional, behavioral, and trauma-related needs and concerns.
- Coordinate with internal and external team members, document pertinent information to facilitate care coordination, and refer patients to other disciplines and resources.
- Apply knowledge of dementia and psychiatric disorders.
- Educate patients and caregivers to facilitate discharge and provide or reinforce individualized patient and caregiver education.
- Support acute hospital services, including emergency department and psychiatric care.
- Facilitate discharge and contingency planning and provide crisis intervention.
- Apply knowledge of cultural issues and community programs involving Medicaid and Medicare.
