Responsibilities
- Evaluate patients' speech, language, communication, and swallowing needs in the home setting.
- Develop and provide individualized therapy interventions based on patient goals.
- Monitor patient progress and adjust treatment plans as appropriate.
- Educate patients and caregivers on therapy recommendations and strategies.
- Complete timely clinical documentation and communicate patient progress with the care team.
Requirements
- Current BLS certification.
- Experience working with geriatric and orthopedic patient populations.
- Home Health work setting experience or readiness to provide care in patients' homes.
- OASIS documentation skills.
- Epic charting experience preferred.
