Responsibilities
- Coordinate patient services across the care continuum.
- Evaluate patient needs and support appropriate care delivery.
- Collaborate with clinical teams, patients, families, and other stakeholders.
- Review care progress, support transitions, and promote safe, efficient outcomes.
- Maintain accurate documentation and follow applicable professional and organizational standards.
Requirements
- Epic charting experience required.
- Acute hospital work setting experience.
- Case management and utilization review skills, including appeals and denials, care coordination, disability case management, discharge planning, disease management, needs assessment and DME ordering, plan of care, and workers’ compensation case management.
- Knowledge of CMS, the Department of Health, DRGs, HIPAA guidelines, OSHA, The Joint Commission, Core Measures, National Safety Goals, and workers’ compensation requirements.
- Acute hospital specialty experience in the Emergency Department, ICU, and Surgical Services is required.
- Medical-surgical experience.
- Completion of the required extra checklist.

Experience
- At least 2 years of experience.
- Two years of acute medical Care Manager experience performing care coordination is required.
- Recent acute medical Care Manager experience within the last 3 months is required.
- Travel nursing experience is required.
- Trauma Level I experience preferred.
- Trauma Level II experience preferred.
