Responsibilities
Support patients and care teams through organized case management, care coordination, discharge planning, and individualized planning of care. Help evaluate patient needs, coordinate appropriate services and equipment, and promote safe transitions across the continuum of care.
Requirements
- Experience working in an acute hospital setting
- Case management and utilization review skills, including appeals and denials, care coordination, disability case management, discharge planning, disease management, needs assessment and DME ordering, plan of care development, and workers’ compensation case management
- Required experience with Emergency Department, ICU, and Surgical Services; MS experience is also included
- Working knowledge of CMS, the Department of Health, DRGs, HIPAA guidelines, OSHA, The Joint Commission, Core Measures, National Patient Safety Goals, and workers’ compensation requirements
- Epic charting experience
- Completion of the required additional skills checklist

Experience
- At least 2 years of acute medical Care Manager experience performing care coordination
- Recent qualifying experience within the last 3 months is required
- Travel nursing experience is required
- Trauma Level I experience preferred
- Trauma Level II experience preferred
