Join our team as a Licensed Clinical Social Worker in Burbank, CA, and support patients and caregivers during acute hospital care in a travel contract role.
Position Details

Responsibilities
- Support patients and caregivers as they navigate social-service needs during hospitalization.
- Contribute to individualized clinical planning and help address patient and caregiver needs.
- Document social-work activity as part of the clinical workflow.
Requirements
- Credential: LCSW or MSW.
- Epic charting system experience is required.
- Skills include care coordination, discharge planning, needs assessment, ordering DME, plan-of-care development, collaborative care, and participation in care conferences.
- Acute hospital experience in Medical Surgical and short-term acute care inpatient medical social work settings.

Experience
- At least 2 years of acute medical social work experience, performing both social work and discharge planning.
- Travel experience is required.
