Responsibilities
- Manage 5150 and other involuntary holds in a psychiatric hospital setting.
- Complete required intake and admission screenings.
- Perform biopsychosocial assessments and conduct initial evaluations.
- Provide required patient and family education, support, and advocacy.
- Work with the multidisciplinary team to support continuity of care.
- Apply knowledge of adoption and foster care, drug and alcohol use and treatment options, and suicide, homicide, and self-harm concerns.
- Apply knowledge of adult protective services, child protective services, emergency detention, guardianship, and required Power of Attorney processes.
- Assess response to treatment and collect and analyze patient information.
- Provide required care coordination and case management services.
- Conduct crisis assessments and mental health assessments, including crisis intervention for teen suicides, drug- and alcohol-related issues, and school violence.
- Provide crisis intervention and management, develop clinical assessment reports, apply ethical principles, identify social, emotional, and behavioral needs, and identify required trauma-related concerns.
- Coordinate with internal and external team members, document pertinent information to facilitate care coordination, and refer patients to other disciplines and resources.
- Apply knowledge of dementia and psychiatric disorders.
- Educate patients and caregivers to facilitate discharge and provide or reinforce individualized patient and caregiver education.
- Support care delivery in emergency department and psychiatric settings.
- Facilitate discharge and contingency planning and provide crisis intervention.
- Apply knowledge of cultural issues and community programs involving required Medicaid and Medicare resources.
Requirements
- Active licensure or qualifying credential as an LCSW, LMFT, or LMHC.
- Required Epic charting experience.
- Ability to perform the clinical, crisis, protective-services, assessment, care-coordination, case-management, education, and counseling functions described above.
