- Schedule: Day shift, four 10-hour shifts from 7:00 a.m. to 5:00 p.m.
- Guaranteed hours: 36 per week
Responsibilities
- Manage 5150 and other involuntary holds, complete intake and admission screenings, and perform biopsychosocial assessments.
- Provide patient and family education, support, and advocacy while working with a multidisciplinary team to promote continuity of care.
- Apply knowledge of adoption and foster care, drug and alcohol use and treatment options, and suicide, homicide, and self-harm concerns.
- Address adult protective services, child protective services, emergency detention, guardianship, and power of attorney matters.
- Assess response to treatment, collect and analyze patient information, and conduct initial evaluations.
- Provide care coordination and case management, including coordination with internal and external team members, documentation that supports care coordination, and referrals to other disciplines and community resources.
- Conduct crisis assessments and mental health assessments, including crisis intervention and management related to teen suicides, drug- and alcohol-related issues, and school violence.
- Develop clinical assessment reports, apply ethical principles, identify social, emotional, and behavioral needs, and identify trauma-related concerns.
- Recognize dementia and psychiatric disorders and support patients in emergency department and psychiatric settings.
- Educate patients and caregivers to support discharge, reinforce individualized education, and facilitate discharge and contingency planning.
- Provide crisis intervention and use knowledge of cultural issues when supporting patients, families, and care teams.
- Apply knowledge of Medicaid and Medicare programs and available community resources.
Requirements
- Licensed Clinical Social Worker with an applicable LCSW, LMFT, or LMHC credential.
- Epic charting experience is required.
- Travel experience is not required.
